Templates are useful when you are working over and over again with same page layout or settings and you can use them as starting point then change settings, picture or anything else to produce something new but with same basic layout.
This depends on the type of image you're attempting to bring into Illustrator.For jpg, bmp, gif, and other pixel images, you can use File > Open > filename.jpg and Illustrator will open a new document with that image as the first object on the page.Or, with Illustrator open (but not maximized) you can grab a file on your desktop and drag it into the illustrator window. If you have a file open, Illustrator will drop the contents of that file onto the artboard as if you copied and pasted it. If you have no illustrator files open, it will create a new illustrator document with that image as the first object on the page. This will also work with PDFs.
Do a google image shirt of "tshirt" and find an image of blank tshirt. Then use photshop or illustrator to add pictures/text as a template. You can find iron-on pages at many stores such as WalMart or Target that will feed through your printer and you can iron them onto a tshirt

It can be open (line) closed (shape) path or image placed in illustrator. Everything that you have inside document in Illustrator like mentioned open, closed paths or images are actually objects you can manipulate, group, aply effect, arrange and so on.
Save the template to your computer, and then open it using any image editing program on your computer. You can use Microsoft paint or another image editing program you bought, or you can download a free program off the Internet. I would recommend using Gimp or Paint.net.
