Are you asking "How is data/files" stored on a spinning magnetic disk within a hard drive, or are you asking how is data/files stored within the flash memory cells of a Solid State [hard] Drive (SSD)? Or, are you asking, "what is a hard disk drive" or "What is inside a hard drive?" Thanks!
you copy all the original files to a another hard disk or flash drive before formatting the disk with the original files
If a program's executable file or setup / install files are not saved to the harddrive, no, the program will not run without the disk in the disk drive.
No, the Disk Defragmenter does not delete any files. The defragmenter rearranges the existing files on the hard drive so they are contiguous, rather than scattered around your hard drive in multiple pieces. The software that deletes temoporary files and what not is called Disk Cleanup.

The Recycle Bin. Windows keeps a special folder, for each disk, for the deleted files; if you open the "Recycle Bin", Windows will show you the deleted files for all connected drives.Note that Windows keeps only a certain number of MB of files for each drive; once that is passed, the oldest deleted files are eliminated permanently, when additional files are deleted.
