A patch is a "band aid" for software. Once the software is created, tested, refined, & tested to the point where it is an official release, fixes may still need to be added.
Many times issues come up that were not anticipated, or the developer may be requested to make the code "compatible" with another program or platform, or any number of situations. Most patches we see today are for security reasons, a cat and mouse game that is always in play.
Programs are released with a revision level like 1.0, 2.0, 3.1 etc. Between the revision adjustments are what is known as the patch. (mostly now referred to as an update.)
When a program goes from say rev. 1.x up to 2.x , this is when all the patches that were added between the rev's are rolled into a released version, which is again patch free, but inevitably will go through the same cycle as the whole environment evolves around it.
