A single piece of software can have hundreds of thousands of lines of computer code. Humans are not perfect - therefore it is possible to miss some errors.

It also takes a lot of time to trawl through each line of code searching for one or two errors. The best solution - is to release the software (so the company gets income from the sales) and release a 'patch' to fix the errors once they're found.

As an example - a person checking 100,000 lines of code - would take over 410 hours to check each line - at an average rate of 15 seconds per line !
