This term refers to the hymen tearing, typically during first-time intercourse.

The hymen is tissue that surrounds the opening to the vagina, it normally doesn't cover much of the vagina and is flexible so actually will usually not tear from vaginal penetration unless the penetration is rough or forced. There is a false idea that when a woman first has sex that the hymen has to be torn, also that there is bleeding and pain as a result, but this is false.

The hymen can be torn long before a woman has sex as a result of masturbation, fingering, rape or sexual assault, using tampons or menstrual cups, and vaginal examinations - the hymen can also remain in tact long after a woman has had sex. Pain and bleeding is uncommon with the hymen tearing, most commonly pain and/or bleeding when a woman first has sex is due to vaginal tearing if the woman isn't fully aroused or relaxed enough to have penetrative vaginal sex.
