Nobody died during an actual mission to the moon.
The astronauts on Apollo 13 were in a position where they would have died except for a massive effort worldwide to bring them safely home. Their craft suffered an explosion in the oxygen tank on the journey to the moon which crippled a number of systems.
In fact, the only deaths to take place in space were the crew of Soyuz 11, Georgi Dobrovolski, Viktor Patsayev and Vladislav Volkov, who were killed after undocking from space station Salyut 1 after a three-week stay. A valve on their spacecraft had accidentally opened when the service module separated, which was only discovered when the module was opened by the recovery team. Technically these are the only fatalities in space (above 100 km).

Neil said" one big step" when they landed on the moon. The scariest part of the moon missions came in Apollo 13. An explosion on the spaceship on the way to the moon made it very difficult for the astronauts to make it safely back to the earth. They were finally able to return safely, but it was very scary for a few days.
