You take it before meals. There should be some kind of indication on a the bottle. The one I take says to take 1 hour before meals. I just started recently, so I have yet to see any results.

You could massage your foot, take Tylenol, or even take Motrin. When taking Tylenol take 2 if you are an adult, for a kid take chewables, 12 and older you could take Tylenol pills.
