Erm, you become really really skinny, lose weight, and then you should start eating unless you want to DIE because your body does not have enough energy; you will lose muscle mass. Once you lose 60% of your muscle mass you will DIE!
In the early stages you get hungry, then you begin to lose energy, then you'll feel cold, you'll eventually lose weight. If you keep it up you might start to run out of key nutritional elements, which can make you sick in various different ways. If you're a woman your menstruation will stop.

Taken to extremes, it can kill you.
