You don't die because you fall asleep. A concussion is an injury directly to the brain which causes the brain to bleed from ruptured blood vessels (similar to when you get a bad bruise on your arm when it gets hit). As it bleeds, pressure builds up inside your skull because there is no place for the blood (and the pressure) to go.

As more and more pressure is put on the brain, it keeps blood from circulating through the brain which causes portions of the brain to start dying. It's not that the person just goes to sleep, the injury and the swelling cause the person to slowly become unconscious which looks like they are asleep. As the swelling continues, the damage to the brain continues to increase until there is so much damage that the brain just stops functioning and the person dies.
