Casting spells is not fantasy since I have done it, though results are another matter. Websites for various types of witches who sincerely believe in spells as a part of their religion list bad spells to cast on anyone, elderly or not. In this case, whether a spell is good or bad is subjective. For instance, casting death spells usually mean bad karma and most consider it unethical. However, if the person is really, truly suffering, then such a spell could be performed in the spirit of love. But the spell would have to truly done in love and not out of hatred, greed, nor other bad motives.

It would probably be more desirable to pray or cast a spell to help the elderly out of their suffering by whatever means the spirits find best and trust that the spirits will choose the best outcome. At any rate, spells and prayers should not be used as a substitute for medical care as the outcomes are never guaranteed.
