It is possible, for those with a weak heart, for someone to be scared to death, in which case an illusion may very well provoke enough stimulation to result in someone's death. Likewise, if you see an illusion and run away without paying attention, you could fall down stairs, off a cliff or into traffic... so there is potential hazard here.
Also, the presence of illusions, or more specifically, hallucinations, may indicate some more serious mental condition including various cancers and tumors. So the illusion may not kill, but it could certainly be a sign of deeper problems.
But, if we're going to be strictly literal, no, an illusion (which by nature is unreal) can in no possible way directly harm (or kill you)
