Your "witch name" or "craft name" as it is more commonly called, is something that you choose for yourself. There are varying methodologies used in the choosing. Some people just randomly assume a name they find appealing, while others incorporate numerology practices into the choosing that allows them to find a name that corresponds with their name number or birth number. Ultimately, it is up to you to figure out what you want your name to be. Many people have multiple names. Some have a "public craft name" that they will use at public gatherings and online as well as semi private name that they use only amongst individuals in their coven and also a private craft name only known to themselves. There are also people that choose to not have any other name than their given name.

Are you perhaps looking for names for characters in a story you intend to write? If anything with "mal" or "mort" in the name might work. If you are however looking to find out if someone is an "evil witch" from their name, I wish you luck... if people had to have names that suited their character then we would have a lot less difficulty choosing our friends.
