This sounds to me like the dreamer has to take care of a young child or a younger sibling. The child requires so much care and attention - is such a big hassle - that the subconscious mind presents the child as an elephant. But unlike a pesky little brother who follows the older brother around, this child requires the caretaker to follow the child.
