Very simply because most toothpastes have "breath freshening" agents which tend to stick to your teeth and gums, and the cold and acidity of the orange juice reacts with menthol that tends to be in these breath freshening agents.

I highly doubt it. If you've brushed your teeth, they become clean. (Or as clean as you make them.) When you drink something after brushing your teeth, it just puts more bacteria in your mouth to rot away at your enamel. I wouldn't say it's "good" but it won't kill you.

It contains citric acid and can erode the enamel of the teeth. If your not drinking enormous of orange juice and brush you teeth reliquary you'll be fine.
