They result from the moving of rock deep under the ground. This movement is caused by the rubbing of one plate into another. The plates have jagged edges, and so when they rub against each other, they get stuck. Since plates constantly move, the plates that are stuck create great tension, and when they are finally free, a huge amount of energy is released. Shock waves move from focus in all directions, and it causes the earth to vibrate.

Definitely. In February 2011, there was the Christchurch earthquake, which killed around 185, making it New Zealand's worst natural disaster in terms of loss of life in 80 years. This was preceded by the Canterbury earthquake in September 2010. Although this earthquake did not cause any loss of life, it was certainly the largest in 80 years to affect a major urban area.

Well, there were alot of cities world wide that experienced strong and large earthquakes in both those years (as there are every year), but, in terms of loss of life and damage (and most likely the most well known events), it would have to go to San Francisco and Oakland, CA.
