Firstly - decide what you need to know, what you will do with the responses, how the responses will be collated, how big a sample you want to find out your answers, time to take to get the answers back who will see the responses. Will the replies be be a "tick box" or have statements from the consumer - if you are new to this - tick box is good

Then design

Start by getting base information from the consumer

age group

gender

marital status

employment status

location of their town /city

Educational status

These are the baseline requirements

Now

in rough format write your questions

number each you do so being aware of grouping relevant questions together

for each option the consumer could reply with allocate a letter

i.e 1. What is you gender

1a Male

1b Female

1c Prefer not to say

2. What is your age

2a 16 - 24

2b 25 - 34

2c 35 - 44

2d 45 - 54

2e 55 - 64

2f 65+

By providing a range of potential answers you will be able to count them on return and gain a percentage of each type of answer given.
