It is essentially carrying out practices that could continue indefinitely. However, you must think about what is being sustained. If you decrease soil fertility or cause pollution issues, that is not sustainable as it will cause problems down the road. If you cannot make a living doing the practice, you won't keep doing it. It's not sustainable. If it is socially inappropriate or damaging, you cannot sustain that. Think slavery. You can harvest a field with slave labor, but that just wouldn't (or hasn't) fly indefinitely. So think environmental, economical, and social sustainability. All three are necessary.
