If they're on the phone and the male is calling the female and she doesn't know who he is, she will probably say, -¿Quién habla? which means Who speaks?, or Who's speaking? You don't generally ask Who's this? when you're "face to face" on the phone. In Spanish, it's a social slur to call someone éste or ésta to their face, or if they're within earshot. She might, however, say Who's this? when they're speaking about a third party; someone in a photograph, for example. In which case, she would ask him, -¿Quién es éste? referring to a male, or -¿Quién es ésta? referring to a female. Gender of the two speakers generally would not affect the conversation, whereas their level of familiarity would.
