Typically it means that the respondent answered key questions in an overly negative fashion to an excessive degree. Caution can identify those cases where a teacher or parent is attempting to portray the child in a highly negative fashion. However, in some cases the child truly is displaying negative behaviors "almost always" or positive behaviors "never".

Generally I tend to interpret Caution or Extreme Caution in the F index as significant frustration on the part of the respondent in dealing with the child on a daily basis. I do not totally discount the ratings because that level of frustration stems from something.
