An MD is a Medical Doctor (Physician). A PhD (Doctor of Philosophy) is the highest level of education particular to many different fields of study (non-medical). For example a PhD in Psychology, or history, or anthropology, etc. An MD is basically a person who has gone through a medical degree and possibly to masters level, and because of this study, they can call themselves an MD (medical doctor). The Ph.D. academically outranks the MD (I consider this unfair given the work an MD has to put in to qualify) as a candidate already has a Bachelors or Masters when they start. The Ph.D. is a research program leading to original findings leading to it's award. This is not normally done during the study to make someone an MD. However, the degree / masters for the MD is a qualifying standard to enable a medic (let's say) to qualify to do a Ph.D, as is the case with any other Bachelors or Masters. These two ways of calling someone a doctor can be confusing to the general public and it can be advised for the Ph.D not to advertise the fact, so as not to cause this confusion. As such, a PhD should only really use 'Doctor' in a professional setting amongst his immediate peers.
