For me it is Cambridge preparation for TOEFL Test. Make sure to have the right edition - I guess it was the forth (for the IBT).I would recommend you to take Kaplan Toefl Practice tests.....it's good and very helpful. There are websites that have study material, practice tests, prep course, online live classes on TOEFL. Online Live classes are worth taking as they are provide classes online and you can clear your queries then and there.

It would have helped to know the name of the university. However, there are many colleges and universities that require the TOEFL for individuals whose first language is not English. If the school makes no mention of it, then I would assume they do not include this as a part of their admissions procedure. Now, many students attempt to bypass the TOEFL and do not indicate that English is not their first language if asked, and this will present problems for the student once they are admitted. I would suggest you contact the institution and inquire whether the school requires a TOEFL anyway, just to make sure. In the long run it will be much better, and you will be able to continue without interruption.
TOEFL is for students who don't have English as their first language. GRE is for English speaking students. You need to take one or the other, not both.
The IBT TOEFL can not be taken more than once a week but repeated as many times as needed to get the score required for the goal set.

No, there are vast discrepancies between the two format. In short, in TOEFL iBT there are Integrated tasks in TOEFL Speaking and TOEFL Writing sections which are missing in PBT. There are also differences in TOEFL Reading and Listening section that basically make the iBT guide book useless for PBT exam.
