The title of "M" is to designate the head of the Britsh Secret Service, a shorthand for "Minister". However, the origins of the name may have come from Ian Fleming deriving it from real-life MI6 head Mansfield Smith-Cumming.

M's real name is Vice Admiral Sir Miles Messervy. In the films, actor Bernard Lee's portrayal of the character was faithful to Ian Fleming's depiction of M in the novels. Rather than changing the name, the head of MI6 would continually be referred to as "M" when someone is promoted to that position. So, Robert Brown, Judi Dench, and Ralph Fiennes share that name.
