You know who is telling the story by the way they talk. If it's first person, they'll say "I" and"we" and "us" when they're "talking" to you. That one's easy. In third person, you have to pay attention to how the writer shows emotion and thoughts. If they're only showing one character, then it's that character telling the story. If they're showing a bunch of people's thoughts and emotions, then the narrator is an omniscient one instead of any of the characters.

Yes, they do count. They count because you can still obtain information and so to say, read the motions, like in a video. There is a story along with video, the motions in the video are "telling" a story. Like music you can read the story through the lyrics. Photographs, you can read the story the photo is telling, like they say, "A photo speaks a thousand words", as to say that there is a background story. Same with a graph, you can still obtain information, but not through words like a book. Sometimes instructions will say "Read the graph to answer the question." it is asking you to search the graph and find the answer.
