It starts with having a good grasp of the basics, and having this as early on as possible. There are many reasons, socially, politically and personally why some students do not or cannot get a strong basic start, but whatever the reason if this doesn't happen, then attempts to jump into the system at various points later on become more and more difficult.

Students in the US for whom English is a second language sometimes have difficulty learning English not because they are not intelligent, but because they never learned the basics of their native language to begin with. We usually assume in the US that when one is learning any language, the basic and general concepts of language and grammar are already known to the student. This is not always the case. Imagine the nightmare of learning the sentence structure, nouns, verbs, and tenses of a foreign language when you have nothing in your experience of language to help you understand what those things are.
