It is quite simple, the citadel means building in or near town and it is a shelter for people in war or (if it exsist at all)during another unpleasent event.The main character, Andrew Manson, is a citadel that defends himself from greed, lust by which he is sorrounded. Other doctors, except Philip Denny, only care for money, they do not follow doctor's vow, and they do not trouble themselves with finding out what is wrog with their patients. Using the metaphore , we can say, that THE WAR-is the doctor's world and the citadel is an ilusion where our hero sheters !
