A current higher than what the plug is rated for will cause that type of condition.
Do not remove any electrical plug by grabbing the wire. This weakens the connection at the plug. ALWAYS grab the plug itself when removing it.
I think it is due to the resistance to the electrical appliance, the resistance offered by the electrical appliance works as a load in the circuit and when load is removed it causes sparking.

Obviously one of the slots on the female connector will remain unused, most likely the neutral slot. The two hot connections and the ground connection would be used.

This is something that should not be done. If this was to be done you would be omitting the ground which in point can make it unsafe. I recommend changing the female end of the system and making it a 4 prong.
