The results were thought to be in Newtons but actually were in pounds-force. This led do incorrect trajectory estimates. The trajectory was thought to be safely above the planet for Mars orbit insertion, but it was much lower. The spacecraft flew through the atmosphere during orbit insertion, causing the spacecraft to be destroyed by aeroheating.

A lesson that could have been learned there was that the English system of measurements must be expunged from the face of the earth (and the rest of the solar system). But it wasn't. NASA still uses a mish-mashes of mixed units. However, there are much bigger lessons to be learned here that apply to all walks of life.

Hubble's problems were due to an incorrectly assembled optical instrument that was used to grind the mirror. Then the same instrument was used to verify that the mirror had been correctly ground.
