The hierarchy is to do with settlement. The bottom of the triangle is the things like Iscolated Houses. There isn't as many of them, and they are so small. Where as at the top of the triangle, there are Capital Cities. There is no canonical "hierarchy of pen color". Institutions which have such a hierarchy will tell you what it is, if it matters. It's generally not a good idea to use such a color hierarchy, since it makes forgery that much easier.
