Practice and read. That's really all you can do.

I suggest (if you don't live in an English speaking country) to always try and speak English to be people that know how to already. It'll practice your ability to perceive sentences together faster and force you to think in the language.

Buy a good book (instead of a vocab builder unless those work for you) and read it. When you come across words you don't know, define it and memorize it.

I also suggest getting a good textbook or grammar book with comparisons in your native language so you can see the similarities and differences.
