Chinese don't typically ask "how are you?" as a follow up to "hello." Their pleasantries are usually "hello" followed by "have you eaten?" (if it is near a mealtime).

Ni hao = hello

Chi fan le mei you? = have you eaten?

If you honestly want to know how someone is (health-wise), you would ask "shen ti hao ma?"
