The pencil had been lost by Jack -- is a passive sentence. You don't have to have the agent (by Jack)

Had gone is the past perfect tense of the verb go. Tenses and voice are not the same thing. Passive voice is created with a form of be and a past participle.

Whenever you see a form of the verb "to be" you must suspect passive voice.

In this case, "was" makes it passive voice. Active voice would be "Molly fixed the car" or "They fixed the car."
