A transition in a story is when the scene or time needs to change. Without some device to smooth the change, the narrative can feel jumpy and disjointed. The writer has to let the reader know immediately that he's taking you somewhere else or that time has passed. A typical example might be "Meanwhile, back at the farm . . .", or "The next morning . . ."

At least that's how this author would describe it. I hope you're not going to cut and paste this into your English homework, because the teacher probably has something specific in mind which you had better look up.
