This is not an idiom. It means just what it says. Someone does have a choice and the choice is something that will bring trouble.
The problem with this sentence is the words "if" and "decide." It should read "The mechanic told me I would have to choose whether I wanted to have the carburetor fixed or I wanted a new car," or " .. choose between having.... or buying a new car."
Because it makes answering the question just SO much easier. Because multiple choice tests provide options for you to choose from instead of you having to answer a question completely on your own. The answer is in front of you, you just have to choose it Also, if you have no ideas which choice is the correct one, at least you have a 1 in 4 chance to get the answer right if you guess and circle one of the four choices.

The Greek root "di" means "two" or "double." It is commonly used in words like "dilemma" (having to choose between two undesirable options) and "dichotomy" (a division into two distinct parts).

Of course you are right. Having two choices can only mean getting to choose twice. But don't let it get you - says the voice of experience! - it's bad for your blood-pressure.

It is ultimately up to the individual to decide what mix of choices and security aligns best with their goals and values.

All nationalities have those among them that have undesirable qualities. However, painting a group as having undesirable traits is stereotyping and would not hold true throughout the society.
You can choose between both. Some doctors recommend having a mixture of both. This provides the health benefits of both worlds.
