Doctor is abbreviated to Dr. when used as a title in conjunction with the person's name, but not when addressing the person by title only or when using doctor merely as a noun, not a title.

For example:

* Dr. Smith * Dr. and Mrs. John Smith * How are you, Doctor? * Margaret is a doctor too.
