You can't actual send your friend a medusa's blood, so basically the only was you can give them one is to go to the store, then at the top click "Exchange Items" and you will be redirected to a page which resembles the Direct Sales, but only for BMI(Black Market Items) instead of horses. Once on the "Exchanged Items" page you will find a tab at the top that says "Manage my item exchange offers," click on that and you will be able to "offer a gift" to your friend.

It means if you have one in your inventory means you can use the one in your inventory if you have one you click in the circle

Some people are willing to buy some black market items for a certain price. check out some player's presentations, see if it says that they will buy your item, and, if you are satisfied with the price, pm them and ask if they would like to buy it. There is also the Black Market Exchange with the Pegasus account on howrse so that you can trade with other players.

first you go to the black market, click on horn of plenty, then click the button next to "use from inventory", and it will say have 1,2 or whatever number you have in your inventory. choose the repicient, and click validate. it will go to a screen that shows what you recieved. im koertchen1 on howrse

No. There is no possible way to remove a BM item on a howrse. You can get rid of the wings from Medusa's Blood by using a Golden Apple, but that is probably the only thing that changes...
if you mean from the black market, go to the thing you have, click on it and then click pay with item from inventory. :D
