This is a standard feature of the Smart, if you leave the car unlocked for more than a few minutes the ignition locks up and a flashing key apears. Just remove the key and press central lock to reset.

It could be caused by many factors, however if it is periodic it is more likely to be a loose connection. If the monitor is a laptop monitor, may be the cable feeding the monitor from the base is worn out and iscausing distortion.
Codes from the Manual:E-1: Temperature is too hot or too cold for the system to work properly. Move your monitor and test strips to a location where the temperature is appropriate and monitor again with a new test strip. You may have to wait for your monitor to adjust to the new temperature. Refer to your test strip instructions for use for the appropriate operating range. If the error message appears again, contact Customer Care.E-2: Monitor error. Turn the monitor off, then repeat previous monitoring steps. If the error message appears again, contact Customer Care.E-3: There may be a problem with the test strip.
