Though that is practically impossible, the Qur'an says that at least he should not leave one hanging for any length of time.

As far as the issue of "approval", i have no idea where people who say that get such a thing. That may be a later invented idea. The Prophet and his companions never asked for "approval" from their wives. why should they? God has already given His "approval". However, also, a man must be smart. If his present wife (or wives) would be overly jealous, and cause too much hardship for themselves, the children or the husband, then the man may wish to think twice. Marriage is serious and should not be entered into lightly.
