It could mean a number of different things. he could believe you are his actual sister, or you he could consider you a close friend, depending on his tone he could also think of you as a jive turkey, or he could be homosexual and think of you as someone who would understand his lifestyle or he might think you are a nun. Not always.. But....

There are certain relationship in which a boy and girl consider themselves as siblings ..this depend on the nature and quality of the boy / girl. The difference is that even though no blood relation ,in this case the boy will have certain privileges over the girl which usually only a bloodbrother will have..!! similarly the girl wil have same respect and love to the boy which otherwise can only be shown to a blood brother.

These are strictly personal and within those two and their family. They may spread the relation to next level like, the kids of the girl ( if she is married ) will start calling him "uncle" just like their real maternal uncle ..means they can keep the relation in such way....

In my experience, only few people will understand this kind of relations..majority of them may laugh at them.. but anyway such relations are possible..but rarely happens..thats all.
