Not if you know where to look. No matter how feminine in any other way, a guy is different from a girl in having an Adam's apple in his throat. I don't know why we have it; as far as I am aware, doctors who perform sex changes can't do anything about it. So, if in doubt, just look at the person's throat.

If you do get intimate a lack of moisture down below and a slightly "hollow" feel to your partner's new private area is an indicator. The best sign I ever found was the "smell" quite laterally of a Ladyboy is much different to a real woman. That is one thing that doesn't change.

A man will always give off a different scent and pheromone to a woman. It's subtle and you need to be very tuned in to spot it. 20 vodkas and a kebab will not assist your sensory perception however.
