This can mean a couple of different things. The first, taken literally, means that the person feels uncomfortable that you are doing too much for them. Maybe you're buying them expensive gifts or doing things they could do themselves, and they feel guilty that they are not doing the same things back to you. The second meaning is more subjective, and is usually said partially in fun and partially in affection -- they're saying you are so good that they feel they don't deserve having you as a friend or lover.
