Lennie loves to pet soft things, such as mice and in this case a dress. He sees a girl with a red dress and because he likes it he reaches out to feel the dress. However, the girl becomes distressed and begins to scream. This in turn distresses Lennie and he "gets all mixed up" and he holds on because that is all that he can think to do. But the girl continues screaming and by this time Lennie is extremely scared so all he thinks to do is hold on. He is extremely strong and when George comes after hearing the girl from a distance he has to hit him over the head with a picket fence to make him let go.
Upon Lennie letting go, the girl reports Lennie and says that she was raped. So people in Weed start a party to kill Lennie. Lennie didn't actually hurt the girl but he did scare her.

The puppy dies from being mishandled by Lennie; in the same way that Lennie kills the mice and the girl.(On the above) Lennie doesn't kill the girl, just mistakes her dress as a furry animal because he has a mental disability. The girl looks at this for rape, but before Lennie can be tried for sexual misconduct, him a nd George run away from the town of Weed into the countryside.
That is debateable. It could be Lennie; as he was the cause for him and George's movement from town-to-town, and for the deaths within the novel. But.. It could be George; as he was the one dealing with Lennie's outputs, and taking the strain for looking after him, and explaining their dream. It's really up to the reader.
