Jack Bell's Market in Kane PA sells Coopers cheese. Think you can order it from his website also. He even sells it smoked - mmm....mmmh.... when we go there, I'm loaded down with it cause so many friends want me to bring it home for them!!
You can buy it at Phillys Finest restaurant in Melbourne off Eue Galle (not sure I spelled that right), plus they have the best cheesesteak sub I have ever had in my life. They use Cooper cheese!

You can really buy it almost anywhere. Walmart deli sells them pre-packaged. However, all European food stores carry them fresh.
Pretty much anywhere, but I don't recommend making your own cheese. There is a high chance of you not making it EXACTLY the right way, and getting you and your family sick. It is also more expensive.

I am not sure why you need to buy melted cheese since you can certainly buy solid cheese and then melt it yourself, however, yes, it is possible to buy melted cheese. In a Spanish or Mexican restaurant, melted cheese is called quesa fondata; in a French restaurant it is called fondue.
