The intoxicating qualities of nutmeg are highly overrated. Consume 2-3 tablespoons of fresh nutmeg (mixed into milk with sugar is nice) and, at some point during the evening, you will feel like you have the flu (hot and fevery), will feel slightly sick to your stomach, will have a headache, and will be unable to fall asleep. Not the effects you were seeking, I suspect. Seek your high elsewhere as this is not worth pursuing.

YES! You crack the outer dark shell, and inside is the nutmeg that you will grate and use. But do not crack the outer shell until you are ready to use it. Out of the shell it will stay fresh for 2 years, but in the shell it stays fresh for 30 years!

You might try mace. However, spices tend to be spices. That means that they really, in my opinion dont substitute that well. Nutmeg is easy to get it seems. Consider the possibility of doing nothing until you can get some fresh nutmeg.

Simple: grate it. If you grate the nutmeg using a microplane, it should have a strong, distinctive odor you can smell from a couple feet away. If it doesn't, it's lost its flavor, throw it away and buy new. It's fairly likely you're going to end up replacing it. I've never seen nutmegs get moldy before, and I live somewhere with 80% humidity. It would have to be stored in some very poor conditions to mold.
