it starts as a yolk in the hen which at the time is called a oocyte which is created by the hens ovary in a process called ovulation. the yolk continues to the oviduct where it can be fertilized (thats if it "can") and then is covered with membrane, structural fibers and layers of albumin (egg white). this part of the oviduct is called magnus. it goes down through the oviduct while continually rotating within the tube. this twists the structural fibers called chalazae which form rope-like strands that anchor the yolk in the thick egg white. There are two chalazae anchoring each yolk, on opposite ends of the egg. the eggshell is deposited around the egg in the lower area of the oviduct just before it is laid. the shell is made from calcite a crystalline form of calcium carbonate. then the chicken will lay her egg from her anus/bottom/bum whatever you want to call it. this entire process takes around 24 hours.
