According to "La Cuisine Anglaise" , you take a small loaf of bread and cut off the two ends, then you take cut out the center of the loaf so that you have to halves about half an inch thick.

Meanwhile you cut a thick steak seasoned with salt and pepper. You place the meat in between the two pieces of bread. You then wrap the sandwich into a letter press for 30 minutes tightening as time passes.
