Sea otters are known to feed on shellfish like clams and mussels by using their teeth to crack open the shells. They balance the shellfish on their chest as they float on their backs and use rocks to break open the shells.

The chest injury may cause blood, fluid and contaminants to pool inside the chest, and placing the injured side lower will minimize accumulation against the damaged lung, the heart and the major arteries.
Placing paddles on the chest externally allows for defibrillation, which delivers an electrical shock to the heart to restore normal heart rhythm in the case of cardiac arrest. Opening the chest to expose the heart allows for direct access to the heart for procedures like cardioversion or internal defibrillation, often done during open-heart surgery.
Meridia's Beacon will randomly appear in a chest, after you reach Level 12. So if you find it in a chest which you believed you already emptied, that's where it came from. After that, it is a Quest Item and can't be removed from your inventory except by placing it in its proper place.
The defibrillator pads are typically placed on the upper right chest area and the lower left side of the chest. These placements help create a pathway for the electrical shock to travel through the heart and restore its normal rhythm when needed. Make sure the chest is clear of any obstructions (clothing, jewelry) before placing the pads.
