There can be several reasons but it is possible you used too much yeast or perhaps the dough was too wet. When making bread it is important to follow the recipe EXACTLY.
Here is a recipe for a white loaf for you to try:-
Place in the machine IN FOLLOWING ORDER
500g (1lb 2oz) of strong (high gluten) white flour sifted with 1&1/4 tsp salt
25g (1oz) of butter
1&1/2 tsp of fast acting dried yeast
300ml (10floz) of warm water in which 2 tsp sugar has been dissolved
Start the machine.

If bread in the bread machine sinks on the top but is otherwise perfect, it is either too much liquid or an improper amount of yeast. Instant yeasts act differently from dry yeasts. If you use instant yeasts, you will need less than if you use dry yeasts. if the bread is otherwise perfect, it is more than likely a problem with the yeast rather than the liquid. Either way, the problem is in the recipe.
