Holden always comments on how much he hates people who are 'phoney' and worrying about ones appearance is something that he would call phoney so when he does this himself it is ironic.
Holden has a strong bond with his sister. He can't commit suicide because for one he doesn't want people looking at his dead body when he has passed away. Holden feels that if he is still here and alive his mother will be fine. If he is dead Holden's mother will go into a state of depression and he doesn't want her to feel like that. Holden loves Phoebe and he doesn't want her to turn out like he has. He is depressed and uses a lot of defense mechanisms because of Allie's death and if he died Pheobe would turn out to be like him.
There would be cameras constantly around you, and your life broadcast to the world. That is only one reason though!!

I would say, being diagnosed with type 1 Diabetes. He actually thought he was going to die. I think being scared for your life would probably be your worst experience. Oh and not to mention he has to live with his worst experience for the rest of his life.

Some would argue that immolation (burning alive) is the worst form of death. Still others would argue that exsanguination would be worst, as it is a lingering form. Some say freezing to death is the worst way of passing. Still others argue cancer, TB, or some other form of disease or illness. I'm certain a few would argue that being devoured by an animal would be the worst, being shot, being stabbed, being beaten, all of these would be the worst. My personal belief is that the worst way to die is not the manner of demise, but the condition; to die alone would be the saddest and the worst way many can conceive to go. To be alone and lonely, to be unloved, uncared for. This is just heart rending to consider.
