Grand Mole does not cause a replay as the effect activates at the beginning of the Damage Step - once the Damage Step is entered, no effect will cause a replay. However since it returns the attacking monster to hand, there's not much point of discussing if it causes a replay or not - a replay is the continuation or discontinuation of an attack if a potential attack target appears or disappears before the damage step. If the attacker has left the field, it's simply not there to do that.

If Number 39 Utopia is attacked while it has no XYZ Material, it will destroy itself in response. This will cause a replay.
