I'm guessing that you're asking if all of the Sailors have a form equivalent to the Eternal form that Sailor Moon attains in season 5 of the anime and in the fifth story arc of the manga. The answer is no; the other Sailors only have the first form they attain and the Super form (which they get about halfway through season 4), which still looks a lot like their first form.

They all join in different episodes. The last scout to join (in the usual [original] five: that means not including Uranus, Neptune, Pluto, Mini Moon, ect.) is Sailor Venus and she joins in the 29th episode in the English version.
Does sailor neptune die?
They all die at one point or another, Saturn dies and is reborn when she destroys Pharaoh 90. Pluto dies in the helicopter when she freezes time. And all of them die when they have their Star Seeds stolen by Sailor Galaxia. They all come back though, along with the other scouts who had their Seeds stolen. :)
